  • With or without any medical conditions

Areas of support provided, include:
Comprehensive assessment and person-centred support planning – including assistance in identifying needs and aspirations, and support in formulating a plan to turn those aspirations into achievable goals.

Self-care skills: including dressing, washing, shaving and toileting.

Household tasks: including cooking and meal planning, housework, shopping, laundry and budgeting.

Social care tasks: including support with benefit payments and money, training in the use of public transport, development of social and recreational activities, and the development of friendships and other supportive relationships.

Educational/vocational development: including support in adult education classes and supported employment placements.

Positive behaviour development: including the development of more positive behaviours and the corresponding reduction of self-harmful or socially inappropriate behaviours which interfere with satisfactory relationships and achievement of personal goals.

Medications management: including supervision of self-administration of medications including those prescribed for mental health problems, and monitoring of beneficial and negative side-effects.
